Do I need to provide anything else?

When making a complaint or request to the council's Data Protection Officer you should provide as much information as possible, including your name, address, phone number, dates/times, location and where the data is held e.g. department name, document title, personnel number or line manager name. If further details or clarification are required you will be contacted directly. Note: failure to provide the required details may have affect the ability of the Data Protection Officer to review your complaint/request.

If you have requested rectification/erasure of your personal data the Data Protection Officer will request evidence of your identity where it is not already held on council systems.
